By registering for and participating in any events listed on MyIota, hosted by Iota Phi Theta Fraternity, Inc., I acknowledge and agree to the following:
1. Assumption of Risk
I understand that participation in this event may involve inherent risks associated with COVID-19, including but not limited to potential exposure to individuals who may carry the virus. By registering for this event, I voluntarily assume all risks related to exposure to COVID-19.
2. Health and Safety Compliance
I agree to follow all COVID-19 safety protocols as implemented by Iota Phi Theta Fraternity, Inc., as well as any applicable local or state regulations. This may include, but is not limited to, mask-wearing, physical distancing, and health screenings.
3. Waiver of Liability
By registering for this event, I, on behalf of myself, my heirs, executors, administrators, and assigns, release, waive, and discharge Iota Phi Theta Fraternity, Inc., its officers, directors, members, volunteers, agents, and representatives from any claims, actions, or liabilities resulting from or related to my participation in this event, including but not limited to exposure to COVID-19.
4. Acknowledgment of Health
By registering, I confirm that, at the time of the event, I am not experiencing symptoms of COVID-19, have not tested positive, and am not under quarantine orders. I agree not to attend the event if I experience symptoms or test positive for COVID-19 prior to or during the event.
5. Indemnification
I agree to indemnify and hold harmless Iota Phi Theta Fraternity, Inc. from any claims, damages, costs, or expenses arising from my participation in the event or breach of this waiver.
By completing my registration for the event, I acknowledge that I have read and understood this waiver and agree to its terms. I understand that my registration serves as a binding acknowledgment of this waiver.
